GENERAL HEALTH INFORMATION
The school nurse, in cooperation with the building principals will see that the following student information is collected. The information shall be on file in the office of the school nurse, with the building principals, and with each teacher in grades K-6:
I. Parent or guardian's name, address, and telephone number.
II. The name, address, and telephone number of the parent(s) or guardian(s), and employer(s).
III. The name, address, and telephone number of a person to contact if school officials are unable to contact the parent.
IV. The name, address, and telephone number of the child's doctor to contact in case of an emergency.
Parents or guardian should not send a child to school under the following conditions:
I. If the child's temperature is over 100°F.
II. Presence of diarrhea in the last twelve hours.
III. Sore throat (inflamed or infected tonsils) accompanied with a fever, coughing, and/or discharge of respiratory phlegm or nasal mucus.
IV. Ear ache with discharge or fever.
V. Diseases such as measles, three-day measles, mumps, chicken pox, or scarlet fever.
VI. Child is feeling uncomfortable and unwell to the extent that the child is unable to accomplish normal school activities.
Children will be excluded from school under the following conditions:
I. Vomiting occurs.
II. Fever of 100°F.
III. Child feels too ill to function well.
IV. An accident occurs at school which requires prompt medical attention.
Ill children will not be sent home alone unless permission is given by a parent/guardian. It is the responsibility of the parents or guardian to assume for the care of a child and to make arrangements for such care.
If the parents or guardian can not be reached at home or at work, the person whose name appears on the emergency information form will be contacted and asked to assume responsibility for the child.
The city emergency unit will be called upon request of the parents or guardian and/or school officials in situations such as fractures, dislocations, etc. If the situation is considered of a serious nature, school officials will immediately call the emergency unit prior to contacting the parents or guardian.
For a student to be excused from physical education or readmitted to physical education following a serious injury or a prolonged illness, school officials, at their discretion, may require a medical doctor's statement.
